Important Notice for Mitsumi FDD Owners
Mitsumi Return Material Authorization Process will change.
Please follow the below instructions to obtain an RMA#.

Effective Date: August 7th, 2000

Return Material Authorization Process

To obtain an RMA#:

1. Download RMA Request form from www.mitsumi.com

2. Print and complete Request form

3.  Fax form to  712-655-3808 (Please note new number.)
(Old number 443-284-1260 is no longer available.)

4.  You will receive an RMA number within 3 days by email or fax.

Shipping Instructions:

1.  Print RMA# on the upper side of the shipping box that contains the FDD.

2.  Ship drive to: Customer Service Center

3.  If the RMA# is not printed on the shipping box, shipment may be refused.

4.  Please package the drive carefully to prevent movement during shipment. Foam cushion is recommended to prevent damage.

5.  DO NOT SHIP THE PRODUCT IN AN ENVELOPE!

6.  Please use an authorized U.S. Postal Service package or equivalent for shipment.

Replacing the FDD: 

1.  Upon receipt of the drive, our Customer Service Center will process and ship a replacement drive to you. Please allow 1 week for replacement, after CSC receives the drive.
